Traveling from I-10 Westbound:
Take the Roosevelt Blvd. exit, exit 361, on the left toward US-17/SR-228/US-17 S. Stay straight to go onto Roosevelt Blvd/US-17 S/FL-15 S. Continue to follow US-17 S/FL-15 S. The street will change names to Park Ave. after passing under I-295. You will approach the clinic on the right, 1 mile past Kingsley Ave.
Traveling from Airport/Northside:
Take 1-295 South towards St. Augustine. Get off at exit 10 for Roosevelt BLVD/US 17/Park Ave. Bear right as you exit. You will approach the clinic on the right, 1 mile past Kingsley Ave.
Traveling from Middleburg:
Head Northwest on US 21/Blanding Blvd.
Turn right at Kingsley Ave.
Turn right on Park Ave.
Clinic will be on the right.
